Start pairing POST

Opens a provider session and returns the pairing; the first QR/code arrives right away when the provider has it, otherwise through SSE `pairing.challenge`. Gates: consent given, relationship not refuted, seat activated, channel enabled, no open pairing. Owner only. The first connection that reaches `paired` starts the free week (SSE `trial.started`).

Fetch a fresh QR/code POST

On demand; the workers also refresh it on a timer and push it through SSE. With `?method=qr`, takes over a pairing the dependent was doing by code on their own phone (the fallback "Vocês estão juntos agora?"): same session, a QR on this screen, the code stops working. Owner only.
